By testing, purchasing or using the neuroVIZR device, I acknowledge and agree to the following terms:

• I have read and understood the waiver.

• I do NOT have epilepsy or any other neurological disease, vertigo or acute ocular diseases.

• I acknowledge the rare risk of seizure-like experiences, even without a history of seizures.

• I am 18 years or older.
• I am NOT pregnant.
• I am NOT currently under the influence of alcohol or drugs.
• I understand that the developer, manufacturer, owner and operator are not liable for any adverse effects.
• I will report any discomfort immediately and the session will be stopped.
