Waiver - Welcome to Your FIRST NEUROVIZR EXPERIENCE

By testing, purchasing or using the neuroVIZR device, I acknowledge and agree to the following terms:

I have read and understood the waiver.
I do NOT have epilepsy or any other neurological disease, vertigo or acute ocular diseases.
I acknowledge the rare risk of seizure-like experiences, even without a history of seizures.
I am 18 years or older.
I am NOT pregnant.
I am NOT currently under the influence of alcohol or drugs.
I understand that the developer, manufacturer, owner and operator are not liable for any adverse effects.
I will report any discomfort immediately and the session will be stopped.

I HAVE READ THE WAIVER AND AGREE TO THE TERMS AND CONDITIONS.

I am happy to be part of this experience and I agree to let VIZR™ use images and sound recordings for advertising, marketing, fundraising, and promotional purposes. We kindly ask for your permission to take pictures and videos of you during the sessions for publication on our social media platforms.
